繼震驚全國的威茲堡命案發生已經過去10個年頭了，當年的幸存者悉妮·普利斯科特（內維·坎貝爾 Neve Campbell 飾）經歷了常人無法想象的痛苦與磨難，如今終于從過去的陰影走出。她將那段不堪回首的往事寫成書，其經歷也被拍成系列電影，一時間紅遍全國。在全國簽售的最后一站，悉妮來到故鄉威茲堡。然而厄運似乎如影隨形，前一晚兩名妙齡女孩在家中即遭鬼臉殺手殺害，悉妮的車廂內更發現了帶血的刀子。警長戴維（David Arquette 飾）對此高度關注，與妻子蓋爾（Courteney Cox 飾）各自積極追查真兇。 與此同時，悉妮的表妹吉爾（Emma Roberts 飾）和朋友們也受到陌生人的死亡來電。新一輪的屠殺就此展開，血雨腥風在所難免……
